Lawrence Kosinski, MD, MBA, AGAF, FACG

Lawrence R. Kosinski, M.D., M.B.A., AGAF, FACG, has been in the practice of Gastroenterology since 1984. He is a managing partner at the Illinois Gastroenterology Group (IGG), the largest Gastroenterology practice in Illinois.

In addition to his medical practice, Dr. Kosinski recently completed a three year appointment as Clinical Private Practice Councilor for the American Gastroenterological Association and served on its Governing Board. Locally, he is a Member of the Advocate Sherman Hospital Board of Directors. In January 2014, Dr. Kosinski started SonarMD, LLC; a technology company formed to bring the success of Project Sonar to a national presence. He serves as its president and CEO.

Project Sonar (PS) is a care management program developed by community-based physicians in partnership with a major payer to improve the management of patients with chronic disease. The key to the success of PS is the combined use of evidence based medicine coordinated with proactive patient engagement. The goal of PS is to move physicians from a dependency on fee for service medicine into value based practice. The initial chronic disease category chosen by PS was Inflammatory Bowel Disease (IBD), a family of disorders that are high cost and high risk with a frequency that has been increasing over the past few decades.

Dr. Kosinski received his MD from Loyola University Stritch School of Medicine in 1978, and earned his Masters of Management Degree (M.B.A.) from Northwestern University Kellogg School of Management in 1998.

Dr. Kosinski also serves as a Member of the Board of Directors of Advocate Sherman Hospital.



Daniel P. Ikeda, MD, FCCP
PMA - Pulmonary Medicine Associates (Pulmonary Medicine, Infectious Disease and Critical Care Consultants Medical Group Inc.), Sacramento, CA

Daniel P. Ikeda, MD, FCCP specializes in Pulmonary Medicine and Critical Care Medicine.

After completing his fellowships at the University of California, Davis Medical School, he joined Pulmonary Medicine Associates (PMA). He has also served as a Medical Director with the Sutter Health System.

Doctor Ikeda submitted the CAMP application on behalf of PMA. The COPD and Asthma Monitoring Project (CAMP) is a proposed payment model designed to treat a population of high risk Medicare beneficiaries with COPD and other chronic lung conditions. Care of this high risk population is provided through remote interactive monitoring that brings all the resources to leverage the expertise of a large telemedicine, pulmonary and allergy practice in the acute and chronic management of large populations of patients with COPD, asthma and other chronic lung diseases.

Doctor Ikeda completed his residency at the University of Hawaii Integrated Medical Residency, and received his Medical Degree from the University of Washington in Seattle.



Frank Opelka, MD, FACS
Medical Director for Quality and Health Policy, American College of Surgeons, Washington, DC

Frank Opelka, MD, FACS is Medical Director for Quality and Health Policy with the American College of Surgeons. Dr. Opelka has chaired the Surgical Quality Alliance, established by the ACS, and the American Medical Association Physician Consortium for Performance Improvement. He also plays a leadership role on the National Quality Forum's (NQF) Consensus Standards Approval Committee and the NQF's Measure Application Partnership.

He also serves as executive vice-president of health care and medical redesign at Louisiana State University (LSU) System, Baton Rouge, where he spearheaded the redesign of Louisiana's health care delivery system, which involved the privatization of a large public hospital system. He also fostered the development of a clinical data warehouse at LSU, which expanded service to a number of national specialty society registries.

He currently serves as the chair of the AMA's Physician Consortium for Performance Improvement, chairs the NQF's Consensus Standards Advisory Committee, and founded and chairs the Surgical Quality Alliance convening over 20 surgical specialties. Dr. Opelka reviews for five international journals and is a published colorectal surgeon, and He a professor of surgery.

Doctor Opelka submitted the ACS-Brandeis Advanced Alternative Payment Model Proposal for a Physician-Focused Payment Model, which seeks to provide novel incentives and tools for both improving the quality of care and reducing costs. The model is episode-based, built on an updated version of the Episode Grouper for Medicare (EGM) software currently used by CMS for measuring resource use.

Dr. Opelka is a native of Chicago, IL, graduated medical school from Rosalind Franklin University of Medicine and Science, Chicago, and completed his surgical residency with the U.S. Army.

Susan Dentzer, a leading national expert in health care and health policy, is the President and Chief Executive Officer of the Network for Excellence in Health Innovation. In her role as President and CEO, Dentzer drives the organization's strategic agenda and priorities, works closely with its members to identify key issues, promotes NEHI's research and policy recommendations, and serves as the organization's spokesperson, providing insightful analyses of the U.S. health care system.

Before coming to NEHI, Dentzer was Senior Policy Adviser to the Robert Wood Johnson Foundation. She previously served as editor-in-chief of Health Affairs, a leading journal of health policy, and as on-air health correspondent on the PBS NewsHour. She is a frequent speaker and commentator on the NewsHour, National Public Radio and other television and radio networks.

Dentzer is an elected member of the National Academy of Medicine and serves on its Board on Population Health and Public Health Practice. She is also an elected member of the Council on Foreign Relations; a fellow of the National Academy of Social Insurance; and a fellow of the Hastings Center, a nonpartisan bioethics research institute. She is a member of the Board of Directors of the International Rescue Committee, and also serves on the Board of Directors of Research!America; is a public member of the Board of Directors of the American Board of Medical Specialties; and is a member of the Board of Directors of the Public Health Institute. She is also a member of the RAND Health Board of Advisors and the Kaiser Commission on Medicaid and the Uninsured.

Dentzer is a graduate and trustee emerita of Dartmouth and formerly chaired the Dartmouth Board of Trustees. She also serves on the Board of Overseers of the Geisel School of Medicine at Dartmouth, earned the institution's Alumni Award, and holds an honorary Dartmouth master's degree. She was also a Nieman Fellow at Harvard University in 1986-87.
