On Wednesday, May 21, 2008 and Thursday, May 22, 2008 the Colloquium will offer a Special Post Conference Session: Chronic Care Professional (CCP) Certification Training Overview. Population health improvement is a new model of care that requires new competencies. CCP certification is the only nationally recognized population health improvement, disease management and health coaching certification program. More and more DM organizations, health plans, and health organizations are requiring, recommending or requesting CCP Certification for their DM, case management, and health coaching professionals.
This CCP post-conference session features a blended learning approach. Participants attend the live session which overviews foundations of population health improvement and combines active learning experiences to build critical patient partnering, behavior change, and health coaching competencies. Following participation in the live program, attendees complete the self-directed eLearning program and online certification examination, on their own, outside of the session. The entire blended learning program is 40-hours in length.
