MEDICAL HOME TRAINING PROGRAM OVERVIEW
The National Medical Home Summit is offering a Medical Home Training Program in the form of an optional course within the conference for attendees wishing to take a deeper dive into the Summit's subject matter.

Medical Home Training Curriculum and Requirements

The Medical Home Training Program curriculum and requirements are as follows:

  • Pre-course reading (6 hours) - Click here to review pre-course reading.
  • Please consider the following preconference questions, which will help frame some of the medical home issues as they relate to your organization - Click here to review preconference questions
  • Complete the online course on Care Management (1 hour). This is module 7 in a group of nine modules offered by the Roger C. Lipitz Center for Integrated Health Care at the Johns Hopkins Bloomberg School of -- -- Public Health, and supported by the John A. Hartford Foundation, for physician practices seeking NCQA qualification to participate in future Medicare demonstrations involving the medical home model. (This module is free to candidates in the Medical Home Training Program; the other eight modules may be purchased here for only $15.00 each.)
  • Attend the Preconference Medical Home Boot Camp (4 hours).
  • Attend the complete Medical Home Summit.
